Kansas City CPO
To Secure the Johnson County, Kansas LPO Certification, please follow the steps below:
1. After your Chadwell University CPO Course, wait 3-4 weeks to receive the PHTA CPO Certification by email from contact@phta.org. Save this pdf certification file.
2. Complete the Johnson County LPO Application and save the completed application.
3. Submit both files, the PHTA certification and the JCDHE LPO application, to JCHD.
Option 1 (Pay by Credit Card): Email the CPO certificate and LPO application to richelle.rames@jocofov.org then call (913) 715-6915 to pay the $110 fee and complete your application.
Option 2 (Pay by Check): Print and Mail CPO the CPO certificate and LPO application to JCDHE, Environmental Division, 11811 S. Sunset Drive, Olathe, Kansas 66061 with your $110 check made payable to JCDHE.
The PHTA CPO Certification is valid for five years.
The JCDHE LPO License is valid for three years.
